This notice explains how your email address is handled when you request an educational handout, subscribe to joint-and-tendon education, or request a separate one-time independent-practice notification.
What is collected
The practice-notification form collects your email address, submission time, and the consent wording shown. The educational-handout form collects your email address, optional first name, requested guide, source or campaign labels, submission time, and consent version. Technical information used for security and spam prevention may also be processed.
Do not submit personal health information. The form is not for symptoms, diagnoses, referrals, appointment requests, health numbers, or medical records.
MSK assessment requests
The private MSK request form collects contact details and limited information about the affected area, duration, diagnosis, imaging, treatments tried, treatment interests, and referral source. It is used by Dr. Yang and authorized clinic staff only to review and route the request and to contact the person about an appropriate appointment. It is not a general medical-record portal: do not upload or enter a health number, referral, imaging file, or detailed medical record.
Submitting the form does not create a guaranteed appointment, establish eligibility, or mean that a procedure is appropriate. Website and campaign fields may be stored with the request so Dr. Yang can understand how patients found the service.
Why it is collected
The one-time practice-notification address is used only for the requested independent-practice update. Educational-handout consent is separate: it is used to deliver the requested guide and occasional evidence-based joint and tendon education. Either choice can be withdrawn. Submitting either form does not create a physician–patient relationship, reserve an appointment, or place you on a clinical waitlist.
Storage and service providers
The website is hosted by Netlify. Netlify processes the website and the separate practice-notification and privacy-request forms. Brevo processes educational subscribers and sends requested handouts and educational emails. Access is limited to authorized account users and service providers acting on this website’s instructions. Information is not sold or provided to other organizations for their own marketing.
Retention and deletion
Practice-notification addresses are retained until the requested notification is sent or withdrawn. Educational subscriber information is retained until you unsubscribe or request deletion. Limited consent and suppression records may be retained as reasonably necessary to respect your choice, demonstrate consent, and meet compliance obligations. Records are reviewed and removed when no longer required.
Your choices
You may unsubscribe from educational emails using the link in each message. You may also withdraw a practice notification, or ask for access to or deletion of information associated with your email address, using the privacy-request form below. Enter only the email address you used and do not include medical information.

Dr. Yang currently provides clinical services as an independent contractor at One Medical Clinic. One Medical Clinic does not own or operate this website and is not responsible for website form submissions.
